All above blanks have options from which you can choose an appropriate option while in description you need to describe yourself about your ad offer.
Example of description-

If you want to apply for this job then you should know the following points:-
i) Article are on technology like android, Windows , Internet . All topics will be provided to you.
ii) You have to provide 3 Article as example of your writing skill and topics of article will be provided to you on your E-mail Address with an example article by me.
iii) There should be 600-800 words in a single article and it should not be copied from any other source while you can read and modify content to write.
iv) Minimum 3 Articles required per day and you can send then to my E-mail Address from any place.
v) As per your writing skills you will be paid between 1000-1500 INR may be more than this amount. All depends on your writing skills.
